APPOINTMENTS and CANCELLATIONS
We do require a 24 hour cancellation notice. There will be a fee applied to your account for any failed appointments. In addition, all questions should be answered 3 business days prior to your child's appointment. Your child’s appointment time is reserved specifically for his/her treatment. Taking extra time to go over questions regarding billing or treatment takes valuable time out of your child’s appointment which may result in the need to reschedule your child's appointment.
INSURANCE
We are in network with Aetna PPO, Blue Cross Blue Shield PPO, Cigna PPO, Delta Dental PPO, Humana PPO, Metlife PPO, and United Concordia PPO. It is always best to contact your insurance and check on your coverage before arriving for your appointment. If you have any questions regarding your EOB's (estimate of benefits), please don't hesitate to contact our office an we will make every effort to go over them with you.
SELF PAY / DIRECT REIMBURSEMENT
All direct reimbursement and self payers are expected to pay in full at the time of service. (credit card, cash, or check)
FLUORIDE
A fluoride treatment is automatically done with every re-care visit. It is the parents/guardians responsibility to know if your insurance covers one or two treatments per year. If your insurance does not cover the second treatment, we ask that it be paid at the time of service. If you only want your child to receive one per calendar year, is it your responsibility to notify us before each appointment. If you have any questions about your insurance coverage, is it your responsibility to contact them and know your policy.
